Using your pet insurance policy is easy.

Visit a licensed veterinarian anywhere in the world.

Step 2:

After you pay your veterinarian for services, download your pet's Nationwide pet insurance claim form and write in the diagnosis and invoice totals.

Step 3:

Fax, email or mail your claim form and veterinary invoice to Nationwide pet insurance. A reimbursement check for eligible expenses will be mailed to you.
